About Our Database
The information on this form is used to maintain the mailing list of the North Carolina Arts Council, which provides free publications, including the NCArts newsletter, and other information about the arts. The constituent information collected is also used by the Council for planning and research and is available on request to the public for arts-related purposes. All recipients of Council information and services are coded using the National Standard for Arts Information Exchange to help identify constituent activities and interests. Sometimes codes are assigned by the Council to make sure information is targeted consistently

We prefer that you submit this information on-line, but if you would like to have a printable version of the database entry form click here. The print form is an AdobeAcrobat .PDF file that will download to your computer, and requires you to open it with the free Acrobat Reader. Click here for instructions on viewing and interacting with PDF files.

Constituents should submit more than one form if they meet the following criteria: Organizations that provide distinct services or programs, such as an arts council that also supports a gallery or a school that also supports a literary magazine (each record will be coded differently), or individuals who have more than one role in the arts, such as an individual artist who is also a contact person for an organization. For assistance in completing this form, please contact the Council at 919/733-2111, or by email to ncarts@ncmail.net.
